I. REGISTRATION: BIRTH – ON TIME

Registration forms are usually provided by and accomplished in the hospital or maternity clinics where birth has taken place, as well as by midwives who deliver babies in various homes or places. (A certified true copy of Marriage Contract is required to determine legitimacy of a child).
